Please complete the Online Job Letter Request Form to apply for a Job Letter from the Human Resources Division, Ministry of Health.
The following instructions should be noted before completing this form.
1. A valid email address is required to complete the form.
2. Complete all sections and required fields as indicated below. Required fields are marked with a red asterisk (*)
3. The option exists to have a copy of the Job Letter emailed to the Institution. This option is ONLY available if the Institution allows receipt of Job Letters in this format.

helpdesk@health.gov.tt5. Public Officers who are paid substantively at other Ministries, are requested to email a copy of their pay slip to and contact their schedule officer.
6. Only one (1) submission per Institution.
