A.  Registration Form
B.  Enrollment Agreement Terms and Conditions
C.  Quran Plan Agreement

Please fill out all the sections, associated questions and fields in this package in its entirety.  Please submit the following items  along with the registration package or after submission by emailing to quran@quba-center.org
1. A recent passport size photo of the student
2. Copy of the birth certificate
3. Check or credit card for for tuition submitted during registration hours

Quran Program:
Monday through Thursday (4 Days/Week)
4:30 Pm to 6:30 PM

Tuition:-

1-One-time Registration Fee -- $50
2-Monthly tuition $225/Quran Program 




Program includes Quran Reading (Noorani Qaida, basic or advanced), Memorization & Salah


WEEKEND PROGRAM:

10:30 AM to 1:30 PM


Saturday AND Sunday ( Two Days/week $350 / Month
Saturday OR Sunday (1 day only) -- $199 / Month
One-time Registration Fee -- $50

Program includes Quran Reading (Noorani Qaida, basic or advanced), Memorization, Islamic Studies, Salah and Activities
