The following information is necessary for you to know and understand before your child may be registered for Childcare activities.

-Please fill out this form for EACH child you have in childcare. You may NOT leave your child(ren) at a childcare setting before this form is completed.
-This form is good for one academic year, and will need to be resubmitted at the beginning of the Fall Semester.
-As the parent or legal guardian of the child, it is your responsibility to notify the Childcare Facilitator of any changes that need to be made for your child(ren)'s form.

-Drop off begins 15 minutes before the advertised event time, and all children must be picked up within 15 minutes of the advertised event end. If the event runs over it is your responsibility to ensure that your children are picked up at the appropriate time.
-When childcare is taking place, the only adults allowed within the arranged childcare space are the scheduled childcare workers.
-You may not drop your child off at childcare unless you are attending the event for which the childcare is offered.
