DAHS Pre-Prom Workshop Parent Contract
This contract must be completed and signed by a parent/guardian of every student attending the DAHS 2024 Senior Prom. Students will not be permitted to attend the Senior Prom without a parent/guardian completing this form.

I understand that this is a school-sponsored event and the use of alcohol, illegal drugs and substances is strictly prohibited at any Levittown Public School Senior Prom. I also understand that all students are prohibited from attending the Senior Prom under the influence of alcohol, illegal drugs and substances. I understand that the Senior Prom is scheduled for June 13th, 2024, from 6:00 PM to 10:00 PM. I understand that if this contract is violated, a parent/guardian will be immediately contacted, and the student will be sent home with a parent/guardian from the Senior Prom and will be subject to further disciplinary action, which may include, but is not limited to: 

Violation of Code of Conduct entered on student record.

I have spoken with my child regarding the content of this contract and his/her post Senior Prom plans. I understand that in case of an emergency, I will be available to pick up my child and/or his/her guest from the Senior Prom and transport them home in a safe manner.
