When you enter the space, fill out this Google Form visitor log with your name, email address and/or phone number, and the day and time you were in the space.
If you bring guests, ask them to fill out this form as well.
The DU board and the Code of Conduct Committee have access to responses and will use this info for contact tracing purposes if somebody reports that they were possibly infectious while at DU. We’ll delete responses after 3 months.
