This form should be completed by any Hub Soccer of Denville player, youth referee, coach or trainer who is unable to pass the TeamSnap Health Check. Hub Soccer of Denville understands there are multiple factors in deciding if it's safe to play soccer. We also understand that the guidance changes often. Therefore, this form was created to ensure we're keeping all players and coaches safe and help them understand when it's safe to return to play.  

Upon receipt of this form, Hub Soccer of Denville will review and provide a return to play date. All decisions on return to play date are made based upon CDC, State of NJ, NJ Youth Soccer and local health department recommendations and guidance.

Please DO NOT participate in any Hub Soccer of Denville events until you are given a return date, as a response to this form. Hub Soccer of Denville is focused on safely allowing our youth to play soccer. We are all in this together and we appreciate your cooperation is keeping everyone safe and healthy!
