The last day of the Fall and Winter Bagel Order for Grades 5-8 is Monday 1/25. This form will cover every monday until Monday 5/3 for Grades 5-8.  Please note that if we are able to have full time learning between now and May we will send out a new form to the Grade 5-8 families to sign up for bagels on the days not paid for.

Group 2- 8 Bagel Days ($16)
Group 3- 8 Bagel Days ($16)

Grade 8
Blue Group- 6 Bagel Days ($12)
Gold Group- Blue Group- 6 Bagel Days ($12)

Every Monday for the dates indicated below, students have the option to purchase bagels for lunch.  

NOTE:  Please fill out a SEPARATE order form for each child.

All payment must come through Venmo. Please make your family’s payment to “@SFDSCACADEMY”. Please be sure to include what children (full name) and class you are making payment for in the notes section. Please note that your order is not complete until payment is made and the order form is submitted.

The completed form(s) and payment is due by Thursday, January 14, 2021  

No refunds will be issued if a child is absent.
