NCKU Exchange Students' Physical Exam 
■Instruction:
Step 1：Fill this google form first.

Step 2：Wait for NCKU health center's email. ( the voucher would be attached with email )

Step 3：Follow the instruction which put in the email.

Step 4：Bring passport, money and the document(download from email and print it out) to the NCKU hospital referral center counter.

Step 5:   After done the exam, please bring the pink receipt to NCKU health center and you'll get the stamp on registration form.

■  Exam Time：(Monday to Friday, no service on weekends and holidays)
AM-- 08:13~11:00 (a.m.)
PM-- 13:03~16:00 (p.m.)

■  No matter which type of exam you choose, please go to the Referral Center in NCKU Hospital Clinic Building (1F) first for paying the money, and then go up stairs (2F) for exam.
