I grant permission to Community Cycles of California (CCofCA) to use photographs and/or video taken of me for use in CCC’s publications such as brochures, newsletters, display boards, and to use such images in electronic versions of the same publications or on the CCofCA web site or other electronic forms or media, and to offer them for use or distribution in other publications, electronic or otherwise, without notifying me. I hereby waive the right to inspect or approve the finished photographs or printed or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the photographs. I hereby agree to release and hold harmless CCofCA and its agents or employees from and against any claims, damages or liability arising from or related to the use of the photographs.
I HAVE CAREFULLY READ THIS CONTRACT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE FROM LIABILITY AND A CONTRACT BETWEEN MYSELF AND COMMUNITY CYCLES OF CALIFORNIA, AND I SIGN IT OF MY OWN FREE WILL. By signing, I agree to abide by the conditions outlined above.
