This form may only be used to add to (or remove players from) an existing roster. DO NOT USE THIS FORM TO SUBMIT YOUR ENTIRE TEAM.

A player's name submitted via this form before game time is eligible for that evening's event (and the rest of the season) only if:
-The player is not already registered on another team.
-The player is not currently playing a Varsity or JV sport.

If one of these stipulations is broken, your team will incur a forfeit.
