PLEASE FILL OUT FORM BELOW AND SUBMIT WITH THE NECESSARY COSTUME PAYMENTS FOR YOUR CHILD. PLEASE READ THE INFORMATION BELOW AND RETURN BY NOVEMBER 6th. By paying for your costume and returning this slip, we understand that you give permission for your child to participate in the year end show.

Important Dates/Terms:  
Please mark these dates in your calendar! Costume payments are due by 11/6. The cost $80 for all Tutu Level Classes and $90 for all Starlet and Prima Level Classes. This will include costume, tights or socks and a participation medal. Students in multiple classes will only receive one participation medal. The payment can be made by check, credit card or cash. The deadline for costume refunds is 11/20.  ***We will not order your dancer's costume(s) if permission slip & costume fee(s) are not received by November 6th.***

Recital Tickets *
RECITAL TICKETS FOR THEATER PERFORMANCE- Tickets will be sold online. Ticket limits and pricing will be determined by the venue and current government restrictions (There is a small convenience fee for this service). Once we confirm the venue, we will send more detailed information.

All Accounts Must Be Paid In Full *
April 1st- All tuition balances must be paid in full. Dancers will not be permitted to perform on stage until balance is current by this date.

Alternate Emergency Plans *
IF there are any world events that would prevent our scheduled theater performance, alternative plans will be made. These plans would be dependent on restrictions but will include: outside performances, videotaped performance with no audience on stage, small in class performance or videotaped in class or virtual at home. I understand that NO refunds of costumes/shoes/accessories will be made once the costumes are ordered regardless of what type of recital is held. Tiny Tutus is not responsible if government restrictions will not allow for in person recitals at the time of the scheduled recitals and knowingly am purchasing recital costumes regardless. I also understand that Tiny Tutus cannot guarantee that classes will remain in person if restrictions are put in place.
Costume Dates/Details  
I understand that tentative dates for Recital 2024 are set for Saturday, May 18th and Sunday, May 19th with Rehearsal dates held the same week on two days between Saturday May 11th-Friday, May 17th, but could change depending on confirmed venue (currently looking at Tuesday and Wednesday of that week). I understand that recitals were listed at registration. I understand I may switch my dancer's class in order to perform in the same show as their sibling, and that this must be done BEFORE costumes are ordered. Classes can not be switched after costumes have been ordered.  I understand that costume payments are non-refundable after November 20th. I understand sizing issues with the costumes will be done at my expense. My child will be measured by the studio staff. If I would like to make a request regarding size, I must do so by November 6th. Otherwise, the studio will order sizes based on measurements taken in class as well as costume companies' sizing charts.
