In memory of former employee, Tammy Kempf, the School District of Manawa is taking nominations for the "Heart of Gold" award. Fill out the following form to nominate a school employee for this award.

Email addresses are only collected to send you a receipt of your submission.

If you do not complete the entire nomination in one sitting, you may submit and edit your submission at a later date.
You may edit your nomination form after submission until 3 pm on April 20th.

A person nominated for the Heart of Gold should possess the following characteristics:

COMPASSIONATE
HUMBLE

Special Note - An individual who has received this award in either of the two previous school years is not eligible.
