We have read the convention "Leader Rules & Guidelines" form and agree that our group will be fully cooperative with those standards. We understand that the New York Ministry Network, New York Youth Ministries, any of its officers, employees or volunteers bear no responsibility for the behavior of our group or for any damages or liabilities that may occur at the hotel/s or at the campus of Onondaga Community College || SRC ARENA. We understand that if there is unruly behavior and property damage or if any incidental and related charges incurred, the hotel management will contact our church directly for payment. We understand that it is our responsibility to have MEDICAL AUTHORIZATION and PARENTAL PERMISSION FORMS in our possession during the convention for each teenager in attendance under the age of 18. The undersigned pastor warrants that the church has run background checks and has checked references on all adult participants that will be traveling on the Youth Convention trip when minors will be on such trip, and is aware of no information that would suggest that any of said persons pose a risk of harm to minors. Background checks will be current and will consist of the minimum of a nationwide criminal records check and a National Sex Offender registry check. Reference checks should be with a person who knows of the participant's suitability to be with children and confirm they are not aware of any information that would suggest that any of said persons pose a risk of harm to minors. We have clear leader/student interaction guidelines for protecting minors. We assume all and any responsibility for any situations that result from a violation of those guidelines by one of our leaders. We agree that our group will abide by all the rules and guidelines for the 2020 New York Assemblies of God Youth Convention. I CERTIFY UNDER PENALTY of PERJURY under the laws of the State of New York that the foregoing paragraph is true and correct.

$75/PER PERSON (STUDENTS & LEADERS) UNTIL MARCH 23RD. IF AFTER MARCH 23RD PRICE INCREASES TO $85/PER PERSON UNTIL MARCH 30TH. ANYONE REGISTERING AFTER MARCH 30TH MUST REGISTER ONSITE AT THE SRC ARENA DOORS.

*All pre-orders must be picked up after night services at the SRC Arena Concessions window. The Youth Ministries Office will provide the SRC concession staff with your groups pre-order.
