Please make sure that ALL CONTACT INFORMATION is correct and that you (student) are reachable while you are in Canada. You are responsible for reading and being aware of all the information on this form and on the website.

MAKE SURE YOU FILL OUT 2 FORMS (ONE for Departure AND ONE for Arrival) IF YOU ARE REQUESTING A DROP-OFF AND A PICK-UP.

* Enrolment office to confirm transportation request (approved/denied) by email

* Should the volume of requests exceed our capacity to transport students, we will approve requests on a first-come, first-served basis.
