【個人資料收集聲明】
本聲明列明教區「古道行」工作小組（「我們」、「我們的」）有關處理參加者（以下定義）個人資料的政策及常規。
〈收集資料〉
1.
我們收集參加者們（「參加者」）的資料，用於本聲明內列明的相關用途。參加者包括凡報名丶登記或參與該些由我們舉辦丶協辦或贊助的任何形式丶任何性質不論是戶內或戶外舉行的節目丶工作坊丶會議丶項目或活動等（「活動」）的任何人士。
2.
有意參加活動的人士若未能提供我們要求的資料，我們可能無法為該人士提供相關活動的服務。
〈收集資料的類別〉
1.
我們收集參加者一些個人資料，方法是通過其使用我們的網站或其他我們指定的電子媒體或工具或手機程序，於網上報名參加活動時收集，同時亦通過活動當天的出席紀錄。該些資料包括參加者的姓名丶年齡組別丶電郵地址及聯絡電話（「聯絡資料」）。
2.
我們將於活動期間拍攝照片和／或錄像，參加者的攝影圖像可能因而在當中出現（「照片」）。
3.
我們概不使用”cookies”去收集任何瀏覧我們網站之人士的任何資料。
〈使用資料〉
1.
我們處理聯絡資料的目的，是為籌辦及管理有關活動，以及進行相關的宣傳活動。
2.
相關活動進行過後，我們會繼續保存聯絡資料，方便我們能不時為參加者提供一些他們可能感興趣的訊息，包括我們將舉辦的節目丶工作坊丶項目丶硏討會丶出版的通訊等。任何參加者如不欲接收上述的電子宣傳訊息，可通知我們，以行使其選擇退出接收訊息的權利。
3.
照片（不會以參加者的名字作識別）可能發放於我們的網站和／或我們的其他社交媒體和／或出現在我們的宣傳單張丶刋物和／或宣傳材料和／或公教報或由The Catholic Diocese of Hong Kong (天主教香港教區）出版的其它刋物裡，作為宣傳／公布「古道行」計劃及紀錄之用。
4.
除非獲得參加者的明確同意，或是獲得該些未滿十八歲的參加者其父母或監護人的明確同意，我們概不將通訊資料及照片用於非上述列明的其它用途。
〈保存資料及安全性〉
1.
收集得來的參加者個人資料會儲存於我們資料庫的系統裡，並經由教區「古道行」工作小組授權的有關組員負責處理，該些資料將予以保密。
2.
上述資料將予以保存直至原來收集該些資料時的目的已不再適用，屆時我們將銷毀該些資料。一般而言，照片會存檔於我們的電腦系統內為期六個月，從相關活動的舉行日期起計算。
〈披露資料〉
1.
我們可能向第三方例如就有關製作活動的市場推廣活動及宣傳材料和／或為建立或維護我們的網站及其它社交媒體而獲我們任命／或授權的代理人丶服務供應商及承包商，提供任何的照片（不會以參加者的名字作識別）。
2.
我們可能跟公教報或由The Catholic Diocese of Hong Kong (天主教香港教區）出版的其它刋物共用任何的照片（不會以參加者的名字作識別），作為宣傳／公布「古道行」計劃及紀錄之用。
3.
除上述説明之外，我們不會將收集得來的參加者任何個人資料向任何外部團體或組織披露，除非獲得參加者的明確同意，或是獲得該些未滿十八歲的參加者其父母或監護人的明確同意；和／或在法律的許可或要求下。
〈查詢及更改資料〉
參加者有權（i）查核我們是否持有其資料及查閱該等資料；及（ii）要求我們改正有關其任何不準確的資料。參加者應以電郵方式

Personal Information Collection Statement
This Statement sets out the policies and practices of Diocesan Working Committee for “Following Thy Way” (“we”, “us”, “our”) in relation to handling of personal data of Participant (hereinafter defined).
Collection of Data
1.
We collect the data of participants (“Participant”) in connection with the purposes set out in this Statement. Participant includes any person signing up, enrolling or joining in programmes, workshops, meetings, events or functions etc. of any type any nature, indoors or outdoors, organized, co-organized or sponsored by us (“Activity”).
2.
If the data requested by us is not provided by a person who intends to join the Activity, we may be unable to provide services to such person relating to the Activity.
Kind of Data Collected
1.
We collect certain personal data of Participant being provided through his/her use of our website or our other designated electronic media/tools or mobile apps while signing up the Activity online, and from attendee records of the Activity. Such data includes Participant’s name, age group, email address and contact telephone number (“Contact Information”).
2.
We will be taking photographs and/or video during the Activity in which photographic image of Participant (“Photo”) may appear.
3.
We do not use cookies to collect any information of any person visiting our website.
Use of Data
1.
Contact Information is processed for the purposes of organizing and managing the relevant Activity and running the related promotions.
2.
Contact Information after the relevant Activity is kept for the purposes of sending communications from time to time to Participant, including our programmes, workshops, events, seminars, newsletters etc. that may be of interest to Participant. Any Participant who does not wish to receive electronic promotional materials as aforesaid may exercise his/her opt-out right by notifying us.
3.
Photo, which is not identified by Participant’s name, may be posted on our website and/or our other social media and/or appear on our flier, publications, and/or promotional materials and/or Kung Kao Po or other publications of The Catholic Diocese of Hong Kong (天主教香港教區) for promoting/publicizing “Following Thy Way” project and record purpose.
4.
We will not use the Contact Information and Photo for other purposes not mentioned herein above unless explicit consent to do so by the Participant or by parent/guardian of Participant under the age of 18 is obtained.
Retention and Security of Data
1.
Personal data of Participant collected is stored in our database system and handled by relevant members authorized by Diocesan Working Committee for “Following Thy Way”. Such data is kept confidential.
2.
The said data will be kept until it is no longer required for the purposes for which it was collected/used. It will then be destroyed. Generally, Photo will be kept in our computer server for a period of six months from the day of the relevant Activity.
Disclosure of Data
1.
We may disclose any of the Photo, which is not identified by Participant’s name, to third parties such as agencies, service providers and contractors appointed/authorized by us in connection with production of marketing and promotional materials of Activity and/or for construction or maintenance of our website and our other social media.
2.
We may share any of the Photo, which is not identified by Participant’s name, with Kung Kao Po or other publications of The Catholic Diocese of Hong Kong (天主教香港教區) for promoting/publicizing “Following Thy Way” project and for record purpose.
3.
Save as aforesaid, we will not disclose any personal data of Participant collected to any external bodies or organizations unless explicitly consented to do so by the Participant or parent/guardian of Participant under the age of 18; and/or permitted or required by law.
Access and Correction of Data
Participant shall have the right to (i) check whether we hold data about you and to access such data; and (ii) require us to correct any data relating to you which is inaccurate. Participant should send his/her request concerned to the Chairman of Diocesan Working Committee for “Following Thy Way” via email at
thyway@catholic.org.hk .
In case of discrepancies between the English and Chinese versions, the English version shall prevail.
