Your child is welcome at holiday club, whatever your family circumstances. Some of our funding comes from monies to support children on free school meals in the holidays, so we ask the following question for their anonymous data collection only.

Children will be engaging in arts and crafts or the outdoors, so they should be dressed in clothes that may come into contact with paint or glue or mud!
Parents/carers are warmly welcome to stay during the sessions and coffee and lunch will be provided. Parents/carers who would be able or like to help are also very welcome to do so :-)

Submitting this registration form accepts the following:

I hereby give permission for my child to attend Holiday Fun and to participate in its indoor and outdoor activities (and if they are under 7 a carer will stay with them).

I am aware that any photographs taken at Holiday Fun may be used in displays at church and for future marketing (if you have indicated no photographs of your child, we try to keep them out of general images too).

In the event of any illness or accident requiring emergency hospital treatment, I authorise designated leaders/first aiders to sign, on my behalf, any written consent form required by medical authorities, if the delay required to obtain my own signature is considered inadvisable by the doctor concerned.
