2020 Special Olympics Maine: Penobscot, Piscataquis & Hancock Area                             4x25 Traditional Relay

Session 1: (Schools)
  8:45am-9:15am: Registration for schools begins promptly (head coach only please).
  9:30am- 11:30: All school races in Aloupis Pool
Session 2: (Adults)*
  11:45am-12:15pm: Registration for adults. To limit congestion please do not arrive prior to 11:45.
  12:30pm-1:30pm: Adult Fundamental races in Means Pool
  12:30pm-3:00pm: Adult Traditional races in Aloupis Pool
*If your agency has athletes in the 16-21 age group, they will participate in Session 2 with your team.

To register Athletes in your delegation for the Traditional 4x25 Freestyle Relay, complete the form below.  Do not use this form for Unified Team Relays. This registration form will allow you to register 5 Teams at a time.  Delegations with more then 5 teams will be required to resubmit this form again (there will be a link to "Submit another Response" once you submit this form).  Delegations are required to provide a qualifying score for each team.  Qualifying scores should be obtained by completing the required training and will be used to properly division teams by age and ability.
 
You will need the following information to complete this registration process:
       o HOD Name/Email
       o Team/s with 4 Athletes
       o      Athlete: Gender, Date of Birth
       o Each Team is REQUIRED to submit a qualifying score

At anytime during this process you can always select the "Back" button (bottom of the form, to the left of the "Next" button) to go back and review/edit the data that you completed prior to your current section.  Once this form is completed entirely and submitted (click "Submit" on the last section) the HOD will receive an email (to the email provided) with a PDF confirmation of what was submitted.  Changes to what your Delegation submitted can be made up until 2 weeks prior to the event, the sooner the better! Athletes must have a current Athlete Medical and Consent/Release on file with SOMaine prior to March 11th in order to be properly registered for this event.  To check on your athletes medical and consent information contact Mike Littlefield, at MikeL@somaine.org.  Blank Athlete Medical and Release forms can be downloaded from the State Website at www.somaine.org/sports/athlete-resources/.   Medicals remain current for three years and the release is good forever (or until updated).  The Release form was recently update in 2017.  Any athlete with a Release form signed prior to January 1st, 2017 must submit a new updated consent.
