1. The user group agrees to assume all liabilities for damage which may occur in, on, or about St. Catherine of Siena Academy (SCA) while said persons or groups are using those facilities when damage to property or injury to persons is the result of either the acts, omissions and/or neglect of the users or their agents, servants, and employees.
2. Educational use is the first priority of SCA. Therefore, all rooms will be left in the exact condition which preceded your use and all student/teacher projects/materials/equipment will not be disturbed.
3. The use of the building is restricted to the specific area(s) requested. User groups in unauthorized areas may be asked to leave the building/facility.
4. Requests for facility use should be submitted to SCA, 21 days prior to requested use. No reservation will be made until the application has been approved by the facilities manager.
5. The right to revoke a requisition application at any time is reserved by the school authorities.
6. Deposit, where appropriated, should reach the Facilities Department at least 10 working days prior to scheduled event. School activities have priority over non-school activities. Therefore, time and/or dates are subject to change or cancellation.
7. If the regular day school program is cancelled due to inclement weather, all daytime and evening activities will also be cancelled. Additional custodial costs may apply if snow removal is needed.
8. Organizations who wish to cancel their scheduled activity MUST NOTIFY the Facilities Manager 48 HOURS IN ADVANCE or they will be charged for expenses incurred. (248) 946-4848 ext 113.
9. SMOKING IS PROHIBITED IN ST. CATHERINE OF SIENA ACADEMY AND ON SCA GROUNDS.
10. NO ALCOHOLIC BEVERAGE OR ILLEGAL SUBSTANSTANCES shall be brought into nor consumed on SCA property.
11. No activity shall be permitted in which open flames are used unless approved by the fire marshal and the facilities manager. Use of fireworks will not be permitted.
12. Under no circumstances shall equipment or other property be stored on school property unless approved by the facilities manager. The school assumes no responsibility for properties left on the premises by the applicant.
13. In the event that it is necessary for the custodian to render services to the group beyond those requested, the group will be at the overtime rate for any additional time.
14. Custodians are not authorized to provide school equipment to user groups except as described and approved on the Facilities Request.
15. Use of the kitchen equipment requires that culinary personnel be on duty.
16. Persons or groups who fail to adhere to the policies and regulations established by school shall subject themselves to forfeiture of all privileges of future use of St. Catherine of Siena Academy.
