PUBLICITY RELEASE: I voluntarily consent and authorize the use of my name, photograph, voice, image, likeness, biographical and personal background information, and other personal characteristics and private information by the U.S. Virgin Islands (USVI).  This information can be used worldwide and in perpetuity, in any manner, alone or as a compilation, and for any purpose without further compensation, notice or consent. I explicitly waive any claims for libel, defamation, false light, right of privacy and right of publicity.

PRIVACY: The personal information requested on this form will be used for the purpose of implementing this agreement and to share information with me about the USVI. I hereby consent to collection and disclosure of my information for such purposes.
 
PRIZES: No purchase necessary. Grand prize of airfare and/or hotel accommodation to the USVI is open to legal U.S. residents 18 years of age or older with appropriate documentation. Prizes are redeemable through December 2022. I understand that prizes cannot be transferred, redeemed for cash or substituted and that even though prizes are considered gifts, I should contact my own accounting and/or tax professionals.
 
INDEMNIFICATION AND RELEASE OF LIABILITY: I hereby release, discharge, indemnify and hold harmless the Government of the U.S. Virgin Islands (USVI), its officers, agents, employees or contractors, including respective officers, directors, managers, members or employees ( from and against any and all liabilities, obligations, claims (including negligence), damages, losses, lawsuits, demands, causes of action, costs and expenses (including reasonable attorney’s fees) that I may have now and in the future, no matter how caused, arising out of, or in connection with my participation in sweepstakes and travel to the USVI, including without limitation liability for property or personal injury, damage, loss or death.
 
JURISDICTION: I agree and consent that any claims or disputes will be submitted to the sole jurisdiction of courts located in the U.S. Virgin Islands and that USVI law will apply.
 
I have executed this agreement voluntarily, and agree that it is binding upon myself, my heirs, executors, administrators and representatives.
