Absence Notice Form

Please fill out the request form in order to be excused from an event. ALL rehearsals and performances are mandatory. If you are unable to attend something due to illness, please send us a doctor's note by the date that you return. We want to be sure you are taken care of and are healthy to rehearse.

If you have a school conflict please make sure the teacher/sponsor of the event reaches out to us as well.

If there is a specific circumstance, please communicate.
