To register for the Middle School Haunted Hayride:
1. Complete the following OLM Registration Form by October 2, 2023; and
2. Send in $25 per person via check payable to OLM, in an envelope labeled, "Haunted Hayride-child's name and grade" by October 2, 2023. (No charge for parents/chaperones attending but not riding on hayride/Cornwalk of Horror)
**Complete One Registration Form per student or per parent who is going on hayride events. Price includes hayride, snacks, bonfire, and optional Cornwalk of Horror**

Winding Brook Farm does not have a lot of lighting so a parent or representative is required to stay with each child to help chaperone. What is the name of Parent/Attending Chaperone responsible for your child on the night of hayride?

All students will go on the haunted hayride. Is your child going on the optional Corn Walk of Horror? (outdoor pre-determined trail in the dark through corn and hay with frightening live villains along the way, begins at 8:45 pm in staggered, small grade-level groups. Flashlights are not permitted)
