I, (as Parent/Custodian/Guardian of my child[ren]), and my student (if 18 years of age or older), (“Student”) acknowledge and understand that the Wyoming City School District will provide parents and students the option to attend school during the first semester of the 2020-2021 school year under the District’s Plan for Return.  Additionally, due to the anticipated changing nature of the COVID-19 pandemic, the School District may be required to educate students who initially select in-person, on-site education via the District’s remote learning plan in the event that its school buildings are closed as a result of the pandemic, or in the event that the District is not able to provide in-person, on-site education to every student who selected that option.  

The District’s unique remote learning plan will provide students with an opportunity to access his/her education from District teachers via Zoom live streaming lessons and/or recorded lessons.  This opportunity will involve the Wyoming City School District live streaming and recording lessons provided by its teachers.  Live video shall be made available for the time it is “live,” and recorded lessons will be made available on teacher CANVAS sites.  

These video lessons shall only be utilized by the Student and Parent for educational purposes.  Such videos are private recordings of the District that I/We acknowledge and understand should not be redistributed, shared, posted, copied or otherwise maintained for non-educational purposes.  I/We understand that the videos provided may contain personally identifiable information of other students. This personally identifiable information is protected under state and federal law and I/We agree not to redistribute, share, post, copy or otherwise distribute such information.

In addition, if Student will receive in-person, on-site education, I/We acknowledge that I/We have read, understand, and will adhere to, and assist Student with adhering to, the School District’s safety protocols that will be in place for the 2020-2021 school year, including, but not limited to, social distancing requirements, the use of face coverings, use of protective barriers, completion of a required daily health check, and other safety protocols required by the School District or local, state and federal government.
