This form is a checklist to help you organize and submit your requirements in order to receive recognition from the Emerging Leadership Program. There are two levels of recognition. They are:

1) Emerging Leaders Program Pin
Involvement in student organizations, spirituality events, attendance at leadership slams, and 100 hours of community service.

2) Emerging Leaders Program Medallion
After completing the Pin path, additional involvement in student organizations, a spirituality retreat, leadership coursework, and 200 hours of community service.

The first section will log your Pin requirements. The second will log your additional Medallion requirements.

Some things may count for multiple areas!
