Please fill in this form to let us know your re-enrollment plans for the 2020/21 school year. Submission is required to reserve a seat for your child for the upcoming new school year, and helps us plan for staffing and resource ordering.

Summary of important procedures and requirements:
1) Complete the end of school year requirements including the school clearance.

2) Take your child for the annual health check and submit the completed health form to the school clinic. The Health Form was attached to your registration email and is available at the Registrar's office.

3) For expatriate families only, submit a copy of the parents and student’s passport page that has:
         
          a) a copy of ACR (Alien Certificate of Registration)
          b) personal information/photo
          b) visa type
          d) date of latest arrival in the Philippines

4) Ensure you read the Student & Parent Handbook which is available on the Hub portal (documents section). By proceeding with enrollment, you indicate your acceptance of CIS rules and regulations.

5) Read and acknowledge the Multimedia Release Information and Agreement form, Enrollment Agreements and the School Fees Sheet for SY 2020-2021. Please go to this link to view our fees sheet: http://bit.ly/CISFees2021

6) Pay school fees on or before August 2, 2020. Payments for the reservation and enrollment fees can be done directly to the Cashier or through a fund transfer. Payments that are made through fund transfers shall be dated according to date of receipt of the amount in the CIS Bank account. Please send a copy of the bank statement/advice slip to lcapuyan@cis.edu.ph; or lleona@cis.edu.ph. Kindly indicate the student’s name/number so we can credit the payment accordingly.

The amount credited to our account shall be the same as the billed figure on the Statement of Account. Any transaction charges will be to the account of the remitter. If variance does occur, the Official Receipt will not be issued pending payment of the lacking amount.

To cover the charges (bank commission fees, transfer fees, etc.) of the receiving bank (Bank of Philippine Islands), please add any of the following applicable amounts in every transaction: Peso Account P150.00 | Dollar Account US$10.00
