In applying for admission to the University Center at Elsie Allen High School, I understand that students must fulfill course requirements that are beyond those required by my student’s attendance area school for high school graduation.  Students in the program may have preparatory summer assignments in their Advanced Placement classes.  I also understand that in order for my student to remain in the program they must maintain a satisfactory academic, conduct, and attendance record.

It is important to note that students in the University Center program will be taking courses on the campus of Sonoma State University in their junior and senior years.  Like any typical college campus it has students of adult ages, has a pub available for SSU students in which alcohol is served, and has materials designed for adult reading/viewing for sale in its campus bookstore.  Students are not permitted to buy, use, sell or possess nicotine, alcoholic beverages, controlled substances, or adult reading material.  Failure to comply with this prohibition or with the direction of program or University staff is grounds for removal from the program.  If a student fails to adhere to the regulations of the program, the student will immediately be dis-enrolled from the program and will be required to enroll at once in his/her attendance area high school.

I have read the above and agree to the conditions of the University Center at Elsie Allen High School program.  Furthermore, I grant permission for my son/daughter’s records and information to be released to Santa Rosa City Schools for its use in determining acceptance into the University Center at Elsie Allen High School.
