This sign-up and consent form is for those saints who wish to participate in indoor, in-person Lord’s Table meetings. We plan to meet each Lord’s Day evening at 5:00 pm to 5:45 pm at the Church in Davis meeting hall, with both Chinese and English speaking blended together. By filling out this form, you are indicating that you wish to participate in person with up to 80 people and are willing to accept the risks involved. The responsible brothers in Davis will do their best to provide a safe environment, following all the local county recommendations (i.e. social distancing, use of masks, taking temperatures, etc.). We realize that in spite of all this, some may feel more could be done to be safer. If this is the case, please fellowship with the responsible brother about your concerns. May the Lord cover and bless the church in Davis and all the meetings.

While we understand each individual may have a different level of caution to prevent the virus's spread, we ask each one to bear one another in love when different safety opinions arise during the meetings.
