Complete this form to connect with a WGIRLS volunteer who will try to help you book an appointment for the Covid-19 vaccine. This form is for seniors (60+) and eligible people who are struggling with the tech-heavy process. Please note that there are no guarantees beyond the promise that our volunteers will do their best to secure you an appointment.  

Someone will be in touch over email or by phone to get additional information required to help book your appointment.  We cannot guarantee a date or time but will do our best to work around any absolute standing appointment conflicts you have if you include them in the comments section of this form.  As a reminder, NO volunteer will ever ask you for your Social Security Number on Insurance details.
