I,
the undersigned, acknowledge and state that I have voluntarily applied to
participate in the Capoeira Workshop conducted by Abadá-Capoeira (by this
reference, including its directors, Masters, Mestrandos (as), professors,
instructors, graduated, officers, employees and agents). This activity
includes, but are not limited to, instruction in the techniques and methods
commonly known as capoeira. I am aware and understand that Martial Arts are an
inherently dangerous and hazardous activity. The instruction in the method that
I will receive involves dangerous and hazardous activities. I am voluntarily
participating in instructions in the methods with full knowledge and
understanding that the risks, dangers and hazards of participation include, but
not necessary limited to, the possibility of serious bodily injury and death. I
hereby assume any and all risks of injury or death inherent the methods and
promise to save and hold Abadá-capoeira harmless from any and all liability of
any kind or nature whatsoever which may arise in connection with my
participation in Abadá-capoeira’s instructions regarding the methods. In
consideration for the proposed instruction in the methods of Abadá-capoeira, I
hereby agree that I, my spouse, parents, heirs, assigns, distributes,
guardians, successors-in-interest and legal representatives will not make a
claim against, or file any legal action of any kind or description against Abadá-capoeira for damages,
injury, emotional distress, bodily injury, death or punitive damages due to or
resulting from negligence of Abadá-capoeira in conducting instruction or with
respect to other activities relating to the methods.
I also grant Abadá-capoeira the irrevocable and unrestricted right and permission to take,
copyright the same, to use, reuse, publish and republish audio recordings and
video images, photographic pictures, etc. (“Material”) that Abadá-capoeira had
taken of me or in which I may be included with others, the same in whole or in
part, or composite, or distorted in character or form, without restrictions as
to changes or alterations, made through medium at any location, and in any and
all media now and hereafter known, and for illustration, promotion, art,
editorial, advertising, trade or other purpose whatsoever. I hereby waive any
right that I have to inspect or approve the finished products.
