Group Polo Shirt Orders
Please fill out this form to let us know if you would like to order a red group polo shirt. These can be worn instead of a squirrels/beavers/cubs polo shirt, or when outside the hut on activities. Please could you fill out the below questions to let us know if you would like to order your child a polo shirt. They cost £9.50 for a child's size shirt and £11.40 for an adult size (No charge for leaders/regular helpers) Example photo is below. 

Please pay into the group bank account using the reference '[Child's name], Shirt'. If you are ordering for multiple children please complete this form separately for each child. Please complete by the 6th June and we shall hand the shirts out in the new term.
