*This form is for Athletic Band Instrument Rentals Only.
*Please allow 24 hours for instrument requests to be processed. You will be notified by email when your instrument is ready to be picked up from the Band Office (M-F, 8 am-5pm).
*Instruments are rented for each ensemble (BUMB/BUBB). The rental fee is $25 per ensemble.
*You must sign an Instrument Use Agreement and pay the Instrument Rental Fee when picking up your instrument. Exact cash or check (made payable to Butler University) required for payment.
