Please complete this form to allow the District to have the proper number of meals prepared for distribution to children on Tuesday moving forward during the 2020-21 school year.  Food will be distributed to your child on Tuesday prior to dismissal.  This will allow students to have a meals available to them on Wednesday for Breakfast and Lunch while they receive remote instruction.

Meal Distribution Model:
- Tuesday distribution: Lunch (1 meal); Breakfast (1 meal)
