I declare my participation in the Erasmus + project "Stand up AGAINST extremism II" implemented with the financial support of the European Commission under the Erasmus Plus program.
By submitting the application, I give consent to the processing of my personal data and image according to current guidelines as below:

I give consent to the processing of my personal data (Data) by the "Zielony Słoń" Foundation, ul. A. Struga 63, Lublin 20-709 (Foundation), for purposes related to the organization by the Foundation of the event referred to in this registration (Event) and my presence at the Event in accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46 / EC (General Data Protection Regulation).
The data may be my name and surname, e-mail address, my image (face). I acknowledge that the Foundation is the administrator of the Data it will obtain in connection with my participation in the Event.
The consent that I hereby give to the processing of Data is voluntary and free. I have been informed that I have the right to withdraw my consent to the processing of Data in full or to the extent chosen at any time in any identifiable and captureable manner.
In the event of any doubts of the Foundation regarding the scope of the statement on withdrawal of consent to the processing of Data, the Foundation will adopt an interpretation extending such statement in order to best protect my interests and Data, access to Data, request their correction, verification, transfer and the right to request their removal without undue delay, which, however, does not affect the fact and the lawfulness of processing Data prior to such my decision, lodging a complaint to the supervisory body: Inspector General for Personal Data Protection (from 25/05/2018: President of the Office for Personal Data Protection), ul. Stawki 2, 00-193 Warsaw, when I consider that the processing of Data is unlawful.
The data will be processed for the duration of my participation in the Event and also for a maximum of 5 years (in particular for archiving, evaluation, control and statistical purposes), subject to Image data. The data will be stored at the Foundation's headquarters, with the option of using virtual and electronic storage resources and sources.
