Thank you for your interest in Sensational Cells with Ms. Beth! This a drop-off group for ages 5-9 that explores the world around us using a variety of hands-on activities and a secular approach. It  meets on Tuesdays, starting August 10, 2021, from 10:00 AM - 2:00 PM (snack is included; bring lunch) and ends on May 24, 2022.  To complete your registration, please fill out this form and submit a $50 non-refundable deposit to hold your spot. This is a 36 class series. The cost of the series is $30/class, billed at a flat-rate on a monthly basis and is payable via Paypal, Venmo, Cash App or ACH deposit. There is a one-time supply fee of $35 per child.

By signing my name below, I agree to the following: 1)  I wish to have my child registered and participate in Little Sprouts Science Club - Sensational Cells for the 2021-22 school year and to have them participate in all activities; 2) I release the camp, its directors, counselors, volunteers, and owner of the property, from any liability in connection with my child’s participation in any events and activities of the camp, which includes, without limitation, any liability related to an accident, an injury or illness suffered by my child; 3) I authorize the camp and persons associated therewith to consent to medical treatment for my child, to select the medical personnel, hospitals and/or clinics to treat my child in case of any accident, injury or illness that may occur; 4) In the event of an emergency, I authorize the camp to contact my child’s doctor, to administer first aid, to take my child to a clinic or hospital (emergency room) or to take any other action deemed necessary by the camp or its employees. *
By signing my name below, I acknowledge the following attendance & payment policies: 1) Class will follow Dekalb County Public School System's calendar and inclement weather closures. 2) Should Ms. Beth have to cancel class due to illness, she will provide as much notice as possible via email and will make the best effort to provide a makeup class. 3) We ask that all children be fever & symptom free for a minimum of 24 hours before attending class following an illness. 4) Payment is due on the first day of the month. After the 5th, there is a late fee of $5.00. If delayed beyond the 10th of the month without communication of plan for payment, it may interfere with continuation of participation in the group. 5) Should the need arise, a courtesy 30-day notice is requested before withdrawing your child from class. *
