PLEASE READ: This form is to be used by patrons requesting a donation of tickets for a fundraising or employee appreciation event. Instead of tickets, a ticket voucher will be mailed to you. Our ticket vouchers are good for two Orchestra level tickets to specific Symphony Series, Pops, or Metro concert. The voucher may be limited to a specific season and certain concerts may be excluded. Please note, we do not donate tickets or vouchers for specific concerts.

You will receive your ticket voucher in 2-3 weeks. If your request is not approved or we have any questions, we will contact you by email.
