Please use this form to book your JUNIOR SCHOOL childcare provision for both Before and After School Clubs for the period Tuesday 8th June - Friday 2nd July 2021 only.

For After School Club, please ensure to indicate your intended collection time on each day your child will attend.

If you are unsure of your requirements for the whole period, please just complete the information that you currently require and then submit a further form at a later date. In this instance please make sure to complete section 9 of the form to advise if the information is in ADDITION to the information already provided, or an UPDATE.
