Dismissal Permission Form 2020
If you want your child (Age 9-12) to meet you along the backside of the building, you’ll need to fill-out this dismissal permission form.  Students who are 13+ don't need permission, and thus don't need to complete this form.

We’ll give your student a laminated dismissal pass to attach to their bag. This pass will indicate to our staff that your child has permission to meet you outside. Once the student shows the pass to our staff they will be dismissed and will not be supervised. You'll then pickup your child at the curb along the backside of the building, following the Drop-Off Map.  Our staff will not supervise, so only do this if you are comfortable.

I give LoMastro staff permission to dismiss my child to meet their ride outside. I understand that once my child shows the pass to LoMastro staff they will be dismissed and will not be supervised. I understand I should pickup my child at the curb along the backside of the building, following the Drop-Off Map.
