"What makes a mentor?" Survey
Thank you for donating your time and for taking the initiative to fill out this survey! Results will be shared with members of the Polar Impact network (https://www.polarimpactnetwork.org/). We hope that your input will help us better understand the needs of members of under-represented ethnic/racial minority groups.

Questions are geared towards mentees and mentors. If a question does not apply to you, please leave it blank!

We respect your right to privacy, and ask that you share what you are comfortable with. While the personal information requested helps us better interpret our results, please don't hesitate to use the "prefer not to say" option and eliminate identifying information from your answers.
