University of Bath - Employability Mentor Scheme
Dear Student,

Thank you for taking the time to fill in the form below, even if you are not interested in the Employability Mentoring scheme, so we can compile information for this pilot programme.

Please note: all information provided below will be shared with Randstad, the Non-Medical Help Provider who are assisting us with implementing this scheme.

For those who are signing up to the scheme, this is on a first-come, first-served basis, but we will aim to match as many students with Employability mentors as possible.

Please submit your responses as soon as possible, even if you do not want to meet with your Employability Mentor straight away (more information in Q14 can be provided regarding a start date for your mentoring).

If you need support with filling in this form, please email: disability-supportworkers@bath.ac.uk - and one of our Disability Support Co-ordinators will contact you and help you fill in the form.

Thank you!

Best wishes,
The Disability Service - University of Bath
