As the parent/ guardian of the above said child. I understand that any type of physical activity or athletic camp comes with inherited risks. I am aware of these risks and therefore do not hold the Lima Warriors Semi Pro Football Club nor any of its members responsible for any accident or injury that may occur as result in my child's participation in this camp.
