By completing this registration, I confirm I am the legal guardian to above named child and give permission for my child to participate in the Warchiefs Flag Football Summer Camp. I agree to hold Warchiefs Youth Baseball, and its employees and volunteers including, but not limited to coaches and trainers, harmless from any and all liability. I give permission to WARCHIEFS YOUTH BASEBALL, and/or parties designated by WARCHIEFS YOUTH BASEBALL to photograph and videotape my child to be used in all forms of media for purposes of advertising, promoting and celebrating for no financial compensation. I hereby release WARCHIEFS YOUTH BASEBALL from any and all claims that may arise out of the use of the photographs, videotape and/or name of the person named.
