Supervisor Work Experience Form
Before starting/completing this form, please read the following:
One or more of your employees is currently enrolled in the Work Experience Program at Roosevelt High School. This program is meant to foster and build critical employable skills needed for high school students to use during their secondary and post-secondary educational experiences. To ensure that students build these skills, we require them to do the following to earn elective credits and to be eligible for graduation:

● Maintain employment and work at least 90 hours 
● Participate in mandatory Work Experience meetings
● Complete Work Experience paperwork
● Be evaluated by a supervisor
● Complete an evaluation reflection

Roosevelt High School and the Weld Re5J School district understand that the Work Experience program could not be successfully completed without the help and support of employers like you. We are also aware that
having an employee enrolled in the Work Experience program will add to your workload. Here at Roosevelt, we try to make the time commitment and completion of supervisor evaluations as easy and meaningful as possible. In fact, a majority of the expectations falls to the student who is enrolled in the program. The student will be required to document work hours and provide the school with your contact information. From employers/supervisors we have two requests:

First, we ask that you participate in a digital supervisor evaluations. Second, we ask that you complete the following form sign to indicate that you agree to assist your employee with the participation in the Work Experience Program at Roosevelt.
