Each employee has the right to work in a professional atmosphere that promotes equal employment opportunities and prohibits discriminatory practices, including harassment.  This Confidential Reporting form allows WI-HER staff members and external partners to confidentially report incidents including but not limited to harassment, stalking, sexual or physical assault, trafficking of persons, hate crime, as well as financial abuse and/ or corruption. Please note that this is an internal report and does not constitute a report to law enforcement.

You may choose to submit a report anonymously. All reports will be reviewed and investigated upon receipt in a confidential and professional manner. Depending on the nature of the report, an update will be communicated accordingly.  If you choose to provide your name, email, and any other contact information, WI-HER will follow up with next steps and updates appropriate for the reported incident. Please note that GoogleForms does not collect the IP address of respondents, which protects anonymity of your report.
