You may name up to two other representatives that can earn credit on behalf of your company. These representatives must be a company principal or a hold a jobsite supervisor role. Representatives are subject to HBA approval.

Provide NAME, TITLE and EMAIL

1 - Your company may send multiple attendees to a class, but only one credit can be earned per class/event.

2 - Representative must attend the entire class/seminar to earn credit.

3 - To receive credit for an IBS seminar, proof of attendance must be provided, not proof of sign-up.

4 - Misuse of the Certified Builder logo and/or designation may result in removal from the program and possible suspension of the company's HBA membership.

5 - Upon receipt of this form, HBA will email a checklist to begin the certification process to the email provided above.
