This is an anonymous survey to assess our needs and programs regarding alcohol use in our school district.  Please only fill out this survey if you have at least one student attending in the district. Please only fill out one survey per family.  If you are participating in this survey to receive the $5 giftcard (available to first 60 families) please fill out the survey and  click on the link after the last question to enter your contact information.  Your contact information will not be connected to any of the answers to the survey.
