By filling out and submitting this registration, you agree with all terms; waivers, receipt of the notice of the annual meeting, the exemption from audit and risk assessment, as described on the reverse of the paper registration form, and all club rules as available on our website.

Before playing, youth will also need to provide a "Letter of Permission" from their parents, indicating that the parent or guardian completed the on-line registration with or for the youth and that they agree with all the terms, etc. found in the registration.
