Notification Of Plans to Register for the 2021-2022 School Year
This form is to gather information on your child's plans for attending school next year. This information is used to inform decisions on staffing, transportation needs, and transfer allotment planning to better help your child have a successful next school year. We understand your plans may change prior to the start of the 2021-2022 school year. Please complete by February 12th.
DECLARATIONS OF RESIDENCE ARE VALID FOR ONE SCHOOL YEAR AND MUST BE RENEWED EACH YEAR. Students currently attending a campus due to a 'Declaration of Residence' will need to provide proof of address prior to the start of the next school year.
