IMPORTANT INFORMATION:
These quilts provide a tangible reminder for people needing support during a difficult time physically, emotionally or spiritually.

Before submission of this request, the recipient of the quilt must be advised of the quilt and be willing to accept the prayer quilt. By submitting this form, you are confirming that the recipient has been contacted.

PLEASE check with the recipient that it is ok that we share this prayer request with our congregtation via our public Facebook page. We do not include last names, but it is not a private page and people outside Joy! are able to view these posts.

Delivery of the Prayer Quilt to the recipient is the responsibility of the sponsor. The quilt will be available for pickup immediately following services on the Sunday it is displayed in worship.

A coordinator of the Prayers & Squares Ministry will review the request form and contact you.
