Release and Hold Harmless
By registering your child for this event, you are signing an electronic permission slip:

I, the undersigned, am the parent or legal guardian of the 6th grade participant named below. I give the named participant permission to participate in the 6th Grade Carnival on Thursday, August 18, 2022.

I am aware and understand that there are inherent risks associated with participation in the 6th Grade Carnival and/or the use of the games & activities during the event. I, on behalf of myself and the participant named below, knowingly and freely assume all such risks, both known and unknown, including those that may arise from the negligence of other participants. I, for myself and the participant named below, and our respective heirs, assigns, administrators, personal representatives, and anyone in such similar capacity, hereby release and hold harmless, Piedmont Middle School Parents’ Club, Piedmont Education Foundation, Piedmont Unified School District, and their employees, agents, members, other participants, and officers from and against any and all claims, injuries, liabilities or damages arising out of or related to our participation in the 6th Grade Carnival.

If you have more than one 6th grader, you may add both of their names to the permission slip.

THIS FORM MUST BE FILLED OUT ELECTRONICALLY BY AUGUST 17, 2022 FOR A STUDENT TO BE ALLOWED TO PARTICIPATE
