***IMPORTANT***

All screening sessions are CLOSED for the 2023-2024 School Year.

Please check back late Summer for screening sessions offered for the 2024-2025 school year.

As a reminder, screening sessions are for current 3-5 year old students currently in daycare or preschool or receiving at home care and who live within Lake Forest School District 67 boundaries.

This is not a Kindergarten screening for incoming or current Kindergarten students.

Please take a moment to sign-up for a screening time and complete the brief questionnaire. Screenings are offered monthly and provide families important information about their child's development. If you have additional questions or need to make a change, please contact Brittany Moccia at bmoccia@lfschools.net.

Screening sessions are offered between 9:00-11:00 AM. Once you complete this form, you will be contacted by Brittany, who will confirm your secured appointment time.
