Dear Parents/Carers,
As we rely strongly on photos and videos for both celebrating and evidencing children's achievements, it is important that we have parents'/carers' permission to use children’s images, video and/or media footage . Please complete and return this form so that we can record your preferences. This agreement will remain on file throughout your child’s time at our academy, but may be altered at a parent's/carer's request at any time by re-submitting this form.  If you have any questions regarding media permissions at Shirebrook Academy, please don't hesitate to contact enquiries@shirebrookacademy.org.
